The Coordinated Specialty Care Transition Study: Final Report provides an overview of transition services for clients graduating from Coordinated Specialty Care (CSC). This the second publication from the Continuity of Care Services Following Coordinated Specialty Care study. The final report is a synthesis of findings from an environmental scan of programs and case studies of nine CSC programs. Based on interviews with nearly 90 respondents, these case study sites highlight the variability in transition services after participation in CSC programs, whether they are provided through step-down or extended length models of care. The report describes how services are structured and sustained, how teams facilitate transitions and what challenges they face, and how organizations and states can support transition processes. A review of financing approaches to continued services is included as well as policy challenges and opportunities in continuity of care such as outreach to community based providers, staffing capacity, disengagement and telehealth.
