This is the full instrument for the Survey of Long-Term Care Awareness and Planning, and Appendix A of the report "Data User's Guide for the Public Use File of the Survey of Long-Term Care Awareness and Planning". The questionnaire was developed by drawing on questions from earlier surveys, including the Health and Retirement Study, America’s Health Insurance Plan’s Buyer/Non-Buyer Survey, MetLife’s Long-Term Care IQ Survey, Lake Research Partners and American Viewpoint’s survey of California voters, the Own Your Future survey, and the Hawaii Long-Term Care Commission survey of Hawaii residents.
